How to Submit Your Rebate

  1. Complete the rebate form in its entirety. All information must be legible.
  2. Attach your sales receipt showing purchase of qualifying LEED Brakes products.
  3. Cut out and attach the original UPC/barcode label from the product packaging.
  4. Mail your completed rebate form, sales receipt, and UPC label to:
